Doctors Surgery Consent Form (only required for John Parry Medical Centre. Earl Street Surgery is an alternate doctor’s surgery but does not require a consent form);

Various forms for students wishing to bring a horse onsite or ride horses recreationally. Students wishing to bring a horse onsite will need to inform the College by 27th January 2021. Please click HERE for a Permission to Agist a Horse on Campus over a Closed Weekend or Holiday.

Australian Commonwealth Immunisation Register (ACIR) History Statement – You can obtain this form by telephoning Medicare 1800 653 809 or at www.humanservices.gov.au
Please be aware if your child is not immunised for measles, in the event of an outbreak they will need to be sent home for two weeks.

College Operating Procedures - The College Operating Procedures is a reference guide for parents, students and staff to describe how the college operates and the various expectations of each party. This includes information on school uniform which must be worn to meet OSH standards;
Residential Operating Procedures - The Residential Operating Procedures provides information specifically for residential students and their families; and
College Drug Education Guidelines - The College Drug Education Guidelines.
P&C Committee Meeting Invitation
A P&C Meeting will be held on Monday, 1 March 2021 with an AGM commencing at 2pm and an ordinary meeting to follow. All parents are encouraged to attend (see attached flyer).
